Practice Enhancement Associate (PEA)

Self-Directed professional to work with physician practices and healthcare organizations throughout Central New York, providing education and training and consulting direction for transitioning practices and providers to the Patient Centered Medical Home Model. Will work collaboratively with other professionals to support transition to electronic medical records, work flow and care management changes, and fulfillment of NCQA medical home criteria. Qualified candidates will have several years of successful experience with physician practice management, clinical quality improvement management or healthcare operations management. Preferred candidates will have a clinical and/or case management background, extensive knowledge of medical home requirements and successful experience leading similar efforts. Extensive travel throughout Central New York required.

Manager, Clinical Care Transitions

Seasoned leader to manage the conversion of physician practices, outpatient and inpatient healthcare providers to the Patient Centered Medical Home model throughout Central New York. Will oversee staff and serve collaboratively with contract professionals to achieve conversion to electronic medical records, work flow and staffing transitions and overall care model changes to result in NCQA medical home designation. Qualified candidates will have proven experience in leading similar efforts, demonstrate an ability to serve effectively in a consulting role and have extensive experience managing major projects over a diverse client base and a large geographic area. Preferred candidates will have a clinical operations, healthcare management or practice management background. Extensive travel throughout Central New York required.
